How do I create an entry in the web application?

You must first register in the web application (see "How do I join a company"). Once you have registered, go to the relevant menu item ("First aid book", "Near misses" or "Unsafe acts"). Then click on the "Create entry" button in the top right-hand corner of the page and fill in the fields of the form accordingly.
